Look at the tweaker's table. C7 is optional. So I put an on-off-on DPDT switch that includes a 500pF (C7) cap on one side, and an 82k resistor on the other.

For reverb I've built many of these with the "single valve reverb" method, though my current version is modified. The standard way sounds great though.
